24/7 Emergency Response

Your loved one wears a personal emergency response device at all times. One press connects them to our response network immediately — day or night, weekday or weekend. Fall, medical event, or simply feeling unsafe — help is always one button away.
Weekly RN Visits
A registered nurse checks in on you every week.
A licensed Registered Nurse visits your home weekly to assess health status, review medications, monitor chronic conditions, and update the care plan. Early detection and consistent nursing oversight keep small problems from becoming big ones.
In-Home Assistance
Twice-weekly HHA support for daily living.
A trained home health aide visits twice each week to assist with activities of daily living — bathing, grooming, dressing, mobility, and personal care. Consistent professional support maintains dignity and keeps routines on track.
Medication Management
The right medication, at the right time.
Our care team provides weekly medication review, reminder support, and coordination with your pharmacy and physician. Medication errors are one of the leading causes of preventable hospitalization in seniors — we help eliminate that risk.
Nutrition Support
Eating well is part of staying well.
Weekly nutritional guidance, meal planning support, and coordination with dietary needs are built into your care plan. Whether it's a chronic condition requiring a specific diet or simply making sure your loved one is eating regularly, we've got it covered.

Every Freedom Program client has a personalized care plan developed by our Nursing Supervisor and coordinated across the entire care team — home health aides, the emergency response system, and your loved one's physician. Nothing falls through the cracks.

Medicaid Waiver Coverage Available
Qualified individuals in Wyoming and Nebraska may have the Freedom Program covered at little or no cost through the Medicaid Waiver program. Ask us during your free assessment whether your loved one qualifies.
The national average cost of assisted living is $4,500–$6,000/month. A nursing facility averages $7,500–$10,000/month. The Freedom Program delivers professional care at home for a fraction of the cost.

Your Care Plan Is Built
Our Nursing Supervisor creates a personalized care plan specific to your loved one's health history, conditions, preferences, and goals. Your Freedom Program team is assigned before services begin.
Care Begins
Your emergency response device is activated, your nursing visits are scheduled, and your home health aide visits begin — typically within days of enrollment.
